Case managers meet with clients, work to understand their needs and connect them to the appropriate resources. Their duties may also include completing paperwork and advocating on a client's behalf. Their goal is usually to improve client self-management strategies and quality of life.
Job Responsibilities:
Provide Targeted Case Management services to clients, aimed at improving their current level of functioning in different areas such as home, school/work and the community.
Complete Family Needs Assessment aimed at helping clients identify what community resources would assist in reducing potential environmental stressors.
Research community programs that may assist clients and assist with any linkage and advocacy clients may require to access needed services.
Intern will learn:
-four core competencies of case management: intake, needs assessment, service planning and monitoring and evaluation.
-to assess clients’ needs, develop care plans, and connect them with appropriate services, such as healthcare, housing, or financial assistance.
-how to assist in coordinating services and resources to help clients navigate complex social systems. This includes learning about the different systems of care available to children and families.
-advocacy, ensuring clients' rights are upheld, and navigating complex systems on their behalf.
-and practice different engagement techniques with clients such as motivational interviewing.
-how to write targeted case management notes, and keep accurate and concise records.
Intern will also receive some training in different mental health treatment modalities available to clients in the practice in order to assist in triaging clients.
Requirements:
Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
Ability to gather and compile information in writing.
Ability to monitor, and record client progress against care plan, and make adjustments to plans and services.
Knowledge of community health care and vocational services.
Ability to communicate information to health care professionals and the public over the telephone.
Bilingual (Spanish-Speaking) preferred but not required.
Position Description
NoahNoel, Inc. is a small practice providing mental health services to children and adults experiencing a range of mental health needs of varying intensity. We specialize in Infant/Preschool Mental Health (0 to 5 years old), Maternal Mental Health, Trauma in Children and Adults, and Children with Learning Disabilities and Developmental needs. Throughout the course of treatment, children and families receive a comprehensive mental health assessment, linkage to appropriate supportive services within their community and special education advocacy when needed.
